We are looking for an experienced and passionate Care Partner to join our team and become a trusted link between our clients, families, support workers, health professionals and service providers.
This is a rewarding and varied role combining care coordination, client relationship management, service coordination, Support at Home funding, rostering and community engagement.
About the Role
As a Care Partner, you will take ownership of a portfolio of clients and ensure their services are coordinated effectively around their individual needs, goals and preferences.
You will be a key point of contact for clients and their families, helping them navigate their care journey and ensuring they receive the right support at the right time.
Key Responsibilities
- Build genuine, trusting relationships with clients, families, representatives and support workers.
- Coordinate and monitor services under the Support at Home program and other relevant funding arrangements.
- Conduct client reviews and reassessments to ensure services continue to meet changing needs.
- Develop, implement and review person-centred care and service plans.
- Coordinate support workers, allied health professionals and external service providers.
- Manage and maintain effective staff rosters, ensuring services are delivered reliably and efficiently.
- Respond promptly and professionally to client enquiries, concerns and changing circumstances.
- Work closely with nurses, allied health professionals, support workers and management to achieve positive client outcomes.
- Support clients through changes in circumstances, including hospital admissions, discharge and changes in care needs.
- Monitor service delivery, client satisfaction and funding utilisation.
- Maintain accurate client records, case notes, care plans and documentation.
- Ensure services are delivered in accordance with relevant aged care legislation, standards, policies and funding requirements.
- Identify opportunities to improve client outcomes and service delivery.
- Develop strong relationships with local community organisations, health professionals and referral partners.
- Contribute to quality improvement, compliance and continuous improvement initiatives.
- Participate in occasional community and client visits across the Greater Parramatta region.
About You
We are looking for someone who genuinely enjoys working with people and is passionate about helping older Australians live well at home.
You will ideally have:
- Previous experience in care coordination, case management, client services or rostering within aged care, disability or community services.
- Strong knowledge of the Aged Care sector and Support at Home program.
- Experience with Home Care Packages (HCP) and/or NDIS is highly desirable.
- Excellent communication and relationship-building skills.
- Strong organisational and time-management skills with the ability to manage competing priorities.
- A practical, proactive and solutions-focused approach.
- Strong computer skills and confidence using CRM, client management and rostering systems.
- Excellent written documentation and record-keeping skills.
- The ability to work independently while also contributing positively to a team.
- A genuine commitment to person-centred care, dignity, choice and independence.
- The ability to handle sensitive conversations with empathy and professionalism.
- A current Australian driver's licence and access to a reliable vehicle is desirable.
Qualifications & Checks
A Bachelor degree or Certificate IV, or relevant qualification in Aged Care, Community Services, Disability, Allied Health or a related field is desirable.
You will also require, or be willing to obtain:
- Police Check
- NDIS Worker Screening Check
- Working with Children Check, where applicable
- Current First Aid & CPR
- Relevant COVID-19 vaccination requirements in accordance with organisational and client requirements.
