About the role
We are seeking a talented and experienced General Practice Nurse to join our dedicated Practice team. This is a Part-time role located in Merrylands Stocklands, NSW 2160. As a member of the team, you will play a vital role in providing high-quality patient care and supporting the overall operations of our practice. The position would suit a person who has school age children.
Hours & Days required are: Monday to Friday - 9.30am - 2.30pm

Responsibilities
- Provide premium care experience to our patients that is genuine, compassionate and have a lot of patience
- Deliver follow up care for the client's care plan that has been developed by the (Registered Nurse)
- Meet the safety needs of the client and use equipment safely and properly
- Maintaining accurate patient records and documentation
- Comprehensive health assessment and reviews
- Liaising with multidisciplinary care teams (GPs, allied health, case managers and family members)
- Completing accurate, timely documentation and progress notes
- Providing nursing care and support to patients visiting the practice
- Assisting doctors with examinations, treatments and procedures
- Administering vaccinations and managing online immunisation programs
- Conducting health assessments, health education and chronic disease management
- Ensuring the practice's clinical areas and equipment are well-maintained and compliant with infection control standards
- Collaborating with the multidisciplinary healthcare team to deliver coordinated patient care
What we're looking for
- Minimum 1 year experience working as a General Practice Nurse or in a similar primary care setting
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia
- Strong clinical skills and ability to work autonomously
- Best Practice knowledge essential
- Recall and reminder knowledge essential
- Chronic disease management required (Preparing Care Plans and Health Assessments)
- Excellent communication and interpersonal skills to engage effectively with patients and the healthcare team
- Demonstrated proficiency in maintaining accurate clinical documentation and data entry
- Commitment to continuing professional development and staying up-to-date with nursing practices
Nurse immunisation certificate with experience in childhood immunisations
