The Role:
As our Community Worker, you will deliver community-based support services and implement programs aimed at improving the social, emotional, and practical wellbeing of individuals and families.
Responsibilities:
- Assess clients’ needs and identify appropriate support services in line with individual circumstances
- Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders
- Provide information, guidance, and referrals to community services, including health, housing, and social support services
- Support clients in accessing community resources, programs, and government services
- Liaise with healthcare providers, social workers, and other service agencies to coordinate care and support outcomes
- Advocate on behalf of clients to ensure access to appropriate services and protection of their rights
Skills & Attributes:
- Relevant qualification
- Minimum 1 year of relevant experience
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Demonstrated empathy, patience, and commitment to client wellbeing
- Knowledge of NDIS framework and person-centred care principles (preferred)
