We are seeking experienced and motivated Registered nurse to join our dedicated Community Nursing at Tooleybuc. As the Registered Nurse, you will:
Provide comprehensive, coordinated and person-centered nursing care to individuals and families within their homes and community settings. Provide a client goal focused service to empower them to achieve their health goals
Delivering a broad range of services including chronic disease management, wound and catheter care, palliative care, health prevention, health promotion and carer support. Post acute services such as CVAD management, VAC management, Chemotherapy disconnections, drain management
You will work collaboratively with consumers, carers, General Practitioners and multidisciplinary teams to ensure continuity of care and optimal health outcomes.
