About the Role
As a Care Partner, you will Your Key Responsibilities
- Put participants at the centre of everything you do by working alongside them to develop personalised care plans that reflect their goals, preferences, strengths and changing needs.
- Champion choice and independence by supporting participants to make informed decisions and confidently navigate their care and service options.
- Facilitate and contribute to case conferences and collaborative care planning, ensuring shared decision-making and supported risk-taking are reflected in each participant’s care journey.
- Build strong partnerships with GPs, specialists, allied health professionals, carers and support networks to ensure seamless, coordinated and person-centred care.
- Stay proactive and solutions-focused by reviewing care notes and service data, identifying emerging risks and responding promptly to changes in participant needs.
- Support participants to understand, plan and manage their individualised quarterly budgets, ensuring funding is aligned with their care plan and priorities.
- Monitor budget utilisation and work proactively.
- Maintain high-quality, accurate and timely documentation, ensuring care notes clearly demonstrate participant outcomes and meet the requirements of the Strengthened Aged Care Quality Standards.
- Take pride in maintaining quality and compliance
- Ensure care management claims are accurate and compliant with Support at Home business rules

Key Responsibilities:
- Provide guidance and support to Support Workers.
- Develop and implement individualised care plans for clients.
- Ensure compliance with healthcare regulations and standards.
- Conduct regular assessments and evaluations of client care.
- Collaborate with healthcare professionals, clients, and families to coordinate care.
- Monitor and manage budgets, resources, and client service agreements.
- Drive quality improvement initiatives and promote best practices in home care.
Qualifications and Experience:
- Registered Nurse (RN) or Enrolled Nurse (EN) background with current registration or certification.
- Strong organisational skills.
- Excellent communication and interpersonal abilities.
- Knowledge of healthcare regulations and best practices in home care.
- Proficiency with electronic health records and care management software.
