Allegiance Heart & Home Care is seeking an experienced, compassionate and reliable Community Support Worker to provide regular in-home support to a participant living in Busby, NSW.
This is an ongoing role suited to a support worker who is confident with personal care, can build a respectful and professional relationship with the participant, and is committed to providing consistent, person-centred support.
Monday to Friday: 8:00 am – 9:00 am
Saturday and Sunday: 8:00 am – 9:00 am
Applicants may be considered for weekday shifts, weekend shifts or a combination of both, depending on availability.
Duties may include:
- Personal care, including showering, dressing, grooming and shaving where required
- Supporting the participant with morning routines
- Assistance with toileting and continence care where required
- Meal preparation and assistance with meals
- Medication prompting in accordance with the care plan
- Domestic assistance, including light cleaning, laundry and general household tasks
- Community access, shopping and appointment support
- Social and emotional support
- Encouraging independence, choice and participation in daily activities
- Monitoring and reporting any changes in the participant’s health, wellbeing or support needs
- Completing accurate progress notes after each shift
- Following the participant’s care plan, risk assessments and organisational policies
We are looking for someone who is:
- Respectful, patient and compassionate
- Confident and experienced in providing personal care
- Reliable and able to attend shifts consistently
- Comfortable working independently in a participant’s home
- Professional and able to maintain privacy and confidentiality
- Able to communicate clearly with the participant, family and care team
- Willing to provide both personal care and community-based support
- Able to support the participant in a dignified and culturally respectful manner
Applicants must have:
- Certificate III in Individual Support, Ageing, Disability or a related qualification
- Previous experience providing personal care
- Current First Aid and CPR certificates
- National Police Check
- NDIS Worker Screening Check
- Working with Children Check, where required
- Valid Australian driver’s licence
- Reliable and insured vehicle
- Full Australian working rights
- Smartphone and ability to complete electronic progress notes
- Availability for the advertised shifts
- Experience supporting older people or participants with complex support needs
- Experience with continence care, mobility assistance and community access
- Experience working in home care, aged care or disability support
- Applicants living in Busby or surrounding suburbs
