At Home Subacute provides high-quality, multidisciplinary care to patients in their own homes as an alternative to hospital-based treatment, supporting recovery, independence and quality of life. This 7-day service brings together allied health, nursing, pharmacy and medical teams to deliver flexible care, including home visits, clinic appointments, group programs and telehealth.
As a Social Worker within the team, you will deliver comprehensive psychosocial assessment, intervention and case management across At Home Subacute and Hospital in the Home (HITH) programs. Working both autonomously and collaboratively, you will support client-centred care, goal setting and safe discharge planning to achieve optimal outcomes in the community.
