We're seeking a Clinical Care Manager to join our team and lead the delivery of high-quality, person-centered care across our in-home aged care services.
About the Role
As our Clinical Care Manager, you'll oversee care planning, provide clinical guidance, and work closely with health professionals to deliver holistic support. This role also involves mentoring our care team and ensuring compliance with Aged Care Standards.
What You'll Do
- Oversee clinical assessments, care planning, and reviews for our clients.
- Administer medications, monitor clinical progress, and document nursing interventions.
- Provide wound care, catheter management, and other clinical procedures.
- Support, mentor, and guide care workers.
- Build strong relationships with clients, families, and health professionals.
- Ensure compliance with Aged Care Quality Standards and clinical best practice.
- Management of client invoices and consumer statements in collaboration with the finance team.
- Lead quality improvement, risk management, and incident review processes.
What We're Looking For
- Current AHPRA registration as a Registered Nurse.
- Experience in aged care, community nursing, or home care is essential.
- Minimum 2 years of experience in aged care, community nursing, or home care.
- Strong leadership and mentoring skills.
- Previous team leader experience is desired.
- Excellent clinical assessment and care planning expertise.
- Knowledge of Aged Care Quality Standards and Home Care Packages.
- Valid Australian work rights are required.
This is a part-time position requiring 20-25 hours per week.

Pay: $45.00 – $50.00 per hour
