About the role and where you will be working:
The role focuses on supporting individuals to maintain independence, improve wellbeing, and access appropriate services while navigating complex social, emotional, and environmental challenges. The role identifies client needs and aligns services with available openings within the organisation, enabling independence and choice through option of provider.
Your responsibilities will include but not limited to:

Identify, assess, and respond to risks such as elder abuse, neglect, self-neglect, and mental health deterioration

Maintain current knowledge of local service networks and referral pathways, developing partnerships within the organisation and industry to enhance client outcomes and service accessibility

Deliver crisis intervention and de-escalation strategies in situations involving distress, mental health concerns, or family conflict

Support clients and families to understand and navigate Home Care Packages, My Aged Care, and other government-funded programs

Translate assessment findings into practical, measurable, and outcome-driven interventions through tailored, person-centred care plans in partnership with clients, carers, and families, ensuring goals reflect client preferences, independence, and wellbeing
About you:
