Job Title: NDIS Support Coordinator
Job Summary:
Labour hire and Recruitment is seeking an experienced Support Coordinator to join our team in Melbourne, Victoria. The successful candidate will work with complex mental health clients, providing support and coordination of services to help them achieve their goals. This role will start part-time with the potential to transition to full-time, offering flexibility for remote work.
Skills:
- Strong passion for helping others
- Interest in mental health
- Ability to think creatively and problem-solve
- Excellent communication and relationship-building skills
- Experience in the industry is preferred
Responsibilities:
- Conduct assessments of client needs and develop individualized support plans
- Coordinate services and supports from various providers to meet client goals
- Advocate on behalf of clients with NDIS and other service providers
- Monitor progress towards goals and adjust plans as needed
- Maintain accurate records and documentation
Qualifications:
- Minimum of 3 years experience in a similar role within the public sector or government setting
- Knowledge of the NDIS framework
- Must have relevant qualifications related to disability support or social work
