About the Role
We’re looking for an experienced Part-Time Chronic Disease Management Nurse to join our Cannington Medical Centre.
Working closely with GPs, nurses, allied health professionals and patients, you’ll deliver patient-centred chronic disease management and preventative healthcare services. You’ll empower patients to take ownership of their health, manage chronic conditions and achieve sustainable improvements to their overall wellbeing.
Key Responsibilities
- Prepare GP Management Plans and Team Care Arrangements.
- Conduct comprehensive health assessments to support the early identification of chronic disease.
- Establish practical health goals and action plans with patients.
- Coach and educate patients on sustainable health and lifestyle changes.
- Coordinate referrals and ongoing care with GPs, specialists and allied health professionals.
- Provide appropriate clinical services, including ECGs, spirometry, urinalysis and ABI testing.
- Identify eligible patients and support proactive patient outreach.
- Build strong relationships with GPs and nursing teams to increase patient referrals.
- Maintain accurate clinical documentation and ensure compliance with Medicare requirements.
- Support and mentor other team members in care planning, health assessments and preventative healthcare.
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse.
- Experience in Chronic Disease Management within a general practice or primary healthcare setting.
- Experience preparing care plans, team care arrangements and health assessments.
- At least two years of direct clinical nursing experience preferred.
- A strong understanding of health promotion and disease prevention principles.
- Excellent communication, organisation and time-management skills.
- The ability to motivate patients and encourage ownership of their health.
- A proactive, collaborative and patient-focused approach.
