Your new role:
Join the Community Services Team within Metro South Health@Home and provide high-quality, patient-centred physiotherapy services across the GEMRHiTH and MAPS caseloads within Community and Oral Health Services. Working as part of an interprofessional team, you will:
Deliver evidence-based physiotherapy assessment, intervention, education and evaluation, applying increasingly independent clinical judgement while working within established protocols and under the guidance of senior clinicians.
Manage an allocated community caseload, prioritising clinical and non-clinical responsibilities, maintaining accurate clinical documentation and undertaking care coordination for clients as required.
Work collaboratively with consumers, families, carers, health professionals and external agencies to achieve person-centred goals and optimise health outcomes.
About you:
We are looking for a compassionate and motivated physiotherapist who is committed to delivering safe, high-quality care and working collaboratively within a community-based interprofessional environment. You will bring:
A tertiary qualification in Physiotherapy and current registration with the Physiotherapy Board of Australia (AHPRA), with the ability to apply evidence-based practice and sound clinical reasoning.
Effective communication, interpersonal and teamwork skills, with the ability to engage consumers, families, carers and health professionals to achieve patient-centred outcomes.
Strong organisational, time-management and prioritisation skills, with the ability to manage a community caseload and maintain accurate and timely clinical records.
