Silver Years Support Pty Ltd is an aged care and NDIS service provider delivering in-home and community support services across Logan, Brisbane North and South, Ipswich, Moreton Bay, Redlands and the Gold Coast.
Position Summary
The Registered Nurse (RN) is responsible for the provision of safe, high-quality, evidence-based nursing care to NDIS participants and aged care clients in their homes and communities. The RN undertakes clinical assessments, develops and implements care and support plans, provides clinical oversight and training to support workers, and ensures all care delivery complies with the NDIS Practice Standards, relevant legislation and Silver Years Support policies and procedures.
Key Responsibilities
- Conduct comprehensive nursing and clinical assessments for participants, including health, wellbeing and risk assessments.
- Develop, implement, review and update individualised care plans, health care plans and support plans in consultation with participants, families and the multidisciplinary team.
- Provide direct clinical nursing care in line with scope of practice, including wound care, medication management, continence management, PEG feeding, catheter care and other complex care needs as required.
- Deliver clinical training, supervision and competency assessments to support workers (e.g. medication administration, manual handling, PEG feeding, subcutaneous injections).
- Monitor and evaluate the effectiveness of care and support delivered, and adjust plans as required to meet participants' changing needs.
- Identify and respond to clinical risks, incidents and changes in participant condition, escalating appropriately and completing required documentation.
- Liaise with participants' GPs, specialists, allied health professionals, NDIS Support Coordinators and families to ensure coordinated, holistic care.
- Maintain accurate, timely and legally compliant clinical documentation and case notes in line with organisational and regulatory requirements.
- Support the development and review of policies, procedures and clinical governance frameworks in line with the NDIS Practice Standards and Quality and Safeguards Commission requirements.
- Participate in incident management, restrictive practice reporting, and continuous quality improvement activities.
- Promote a person-centred, strengths-based approach that upholds participants' rights, choice and control.
- Maintain up-to-date knowledge of relevant clinical guidelines, NDIS legislation, and best practice standards.
Key Selection Criteria
Essential:
- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated experience providing clinical nursing care in a community, disability, aged care or home care setting.
- Sound knowledge of the NDIS Practice Standards, NDIS Code of Conduct and relevant disability/aged care legislation.
- Experience developing and delivering clinical training and competency assessments to support workers.
- Strong clinical assessment, critical thinking and problem-solving skills.
- Excellent verbal and written communication skills, including clinical documentation.
- Ability to work autonomously, manage time effectively and travel between client locations.
- Current Australian driver's licence and access to a reliable, insured vehicle.
- Current NDIS Worker Screening Check (or willingness to obtain).
- Current Working with Children Check (Blue Card) (or willingness to obtain), if supporting participants under 18.
- Current First Aid and CPR certificates.
- Evidence of up-to-date vaccinations, including influenza and COVID-19, in line with organisational policy.
Desirable:
- Experience working with participants with complex support needs, including SIL, high physical support needs, or complex health conditions.
- Experience in aged care nursing or community nursing.
- Certificate IV in Training and Assessment (TAE) or equivalent.

- Working with Children Check (Blue Card), where applicable.
- National Police Check.
- Current First Aid & CPR certification.
- NDIS Worker Orientation Module ("Quality, Safety and You") completion certificate.

Pay: $58,037.40 – $110,694.53 per year
