Key responsibilities may include:
- Providing person-centred support based on each participant’s individual needs, preferences and goals
- Assistance with personal care and daily living activities
- Supporting community access, appointments and social participation
- Supporting participants to build and maintain independence
- Medication assistance in accordance with training and participant care plans
- Supporting participants with complex and high-intensity support needs, where appropriately trained
- Following individual support plans, health care plans and risk management strategies
- Maintaining accurate progress notes and required documentation
- Communicating effectively with participants, families and other members of the support team
- Providing High intensity care
Pay: $40.00 – $47.00 per hour
Expected hours: 10.0 – 30.0 per week
