West Coast Medical is a busy, doctor-owned general practice located in South Perth. We are committed to providing high-quality, patient-centred healthcare and are looking for an experienced and compassionate Chronic Disease Management (CDM) Nurse to join our growing team.
This is an exciting opportunity for a motivated nurse who is passionate about preventative healthcare, chronic disease management, and helping patients achieve better health outcomes.
About the Role
Working closely with our General Practitioners and multidisciplinary team, you will coordinate and manage chronic disease programs while providing exceptional patient care.
Your responsibilities will include:
- Coordinating and managing GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
- Preparing and completing Annual Health Assessments, including Aboriginal and Torres Strait Islander Health Assessments
- Coordinating recalls, reminders and preventative health programs
- Conducting chronic disease reviews and patient education
- Liaising with allied health providers, specialists and community services
- Supporting care planning and continuity of care for patients with complex health needs
- Assisting with quality improvement initiatives and accreditation requirements
- Maintaining accurate clinical documentation and Medicare compliance
- Providing clinical support to GPs when required
About You
To be successful in this role, you will have:
- Current AHPRA registration as a Registered Nurse or Enrolled Nurse (Medication Endorsed)
- Previous experience in General Practice, particularly in Chronic Disease Management
- Sound knowledge of Medicare item numbers and care planning requirements
- Excellent communication and organisational skills
- Strong attention to detail and the ability to manage competing priorities
- A compassionate and patient-focused approach to care
- Confidence working independently while also collaborating within a multidisciplinary team
- Current CPR certificate
Experience with Best Practice software and Cubiko reporting will be highly regarded.
