Communify’s Medicare Mental Health Centres (MMHC) are safe and welcoming spaces that provide high-quality mental health supports delivered by a multi-disciplinary team of professionals and people with a lived experience of mental health. Funded by Brisbane North PHN, the centres provide free, integrated support over extended hours for individuals with moderate to severe mental health concerns and complex life circumstances, as well as support for careers and families.
Communify is seeking an experienced and passionate
Mental Health Practitioner to join our innovative
Mental Health Care Partners (MHCP) Pilot Program, delivered through our Brisbane Inner North Medicare Mental Health Centre.
This exciting opportunity allows you to work as part of a multidisciplinary team supporting adults experiencing complex mental health challenges. Working across our Medicare Mental Health Centre, participating GP practices and within the community, you'll play a key role in improving continuity of care through recovery-oriented, trauma-informed and person-centred practice.
About the role
- Provide comprehensive biopsychosocial assessments and develop recovery-oriented care plans tailored to individual needs.
- Deliver evidence-based therapeutic and psychosocial interventions that support participants to achieve their recovery goals.
- Develop collaborative shared care plans with participants, GPs and other health and community providers.
- Deliver support within participating GP practices, as well as outreach services in participants' homes and community settings.
- Coordinate multidisciplinary care, facilitate warm referrals and support seamless transitions between services.
- Assist participants to navigate health and community supports while strengthening their independence and wellbeing.
- Maintain accurate clinical documentation and participant records in line with organisational and legislative requirements.
- Contribute to a collaborative team culture focused on quality improvement, integrated care and positive mental health outcomes.
About You
You are a compassionate, adaptable and recovery-focused mental health professional who is passionate about supporting people experiencing complex mental health challenges. You thrive in collaborative environments, enjoy building strong relationships with participants and stakeholders, and are committed to delivering person-centred, trauma-informed care that empowers people to achieve their recovery goals.
You'll bring
- Tertiary qualifications in Social Work, Psychology, Occupational Therapy, Mental Health Nursing, Behavioural Science, Counselling, Human Services, Community Services or a related discipline.
- Current professional registration or accreditation with the relevant professional body (AHPRA, AASW or equivalent), where applicable.
- Experience supporting people experiencing severe and complex mental health concerns within community mental health, primary care or multidisciplinary settings.
- Demonstrated experience delivering mental health assessments, care coordination and evidence-based therapeutic or psychosocial interventions.
- Strong understanding of trauma-informed, recovery-oriented and strengths-based practice.
- Experience working collaboratively with GPs, multidisciplinary teams and community service providers to deliver integrated care.
- Excellent communication, relationship-building and organisational skills, with the ability to build strong partnerships with participants and stakeholders.
Essential Requirements
- Current Queensland Driver Licence.
- Current National Police Check.
- NDIS Worker Screening and Working with Children Check (or the ability to obtain).
- Relevant professional registration or accreditation where applicable.
