About the Role
East Avenue Medical Centre is seeking an experienced Registered Nurse to join our team as a driving force and advocate for preventative health in our local community. In this rewarding role, you will work closely alongside our General Practitioners and patients to identify, manage, and monitor chronic health conditions, ultimately improving long-term health outcomes and reducing preventable hospital admissions.
While clinical experience in Chronic Disease Management (CDM) is highly regarded, we place immense value on a friendly, proactive, supportive nature and a strong alignment with our collaborative practice culture.
Key Duties and Responsibilities
- Prepare, implement, and review Care Plans and Team Care Arrangements (TCAs).
- Conduct health assessments to assist in the early identification of chronic diseases.
- Work collaboratively with GPs to promote preventative healthcare pathways throughout the practice.
- Empower and coach patients to increase ownership over their health using behavior change principles.
- Coordinate multi-disciplinary team care, enabling patients to access essential allied health services.
- Manage administrative tasks and scheduling related to dedicated CDM and health assessment appointments.
- Deliver home-based nursing care to eligible patients under the direct supervision and delegation of our GPs.
- Actively work to increase the reach of preventative care plans to new and existing patients.
Requirements
- Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) / AHPRA.
- Demonstrated experience in nursing (primary health care or general practice experience is an asset).
- Strong communication skills with a compassionate, patient-centered approach.
