We help participants build independence, stay connected and feel genuinely supported through in-home support, personal care, community participation, transport, daily living, SIL, SDA, STA and ILO supports.
About the Role
We are seeking a experienced, motivated, and compassionate NDIS Service Coordinator to join our team. You will work closely with participants, families, and service providers to coordinate supports aligned with NDIS plans.
Key Responsibilities
- Liaise with service providers, therapists, and stakeholders
- Monitor participant progress and adjust supports as needed
- Maintain accurate records and documentation
- Advocate for participants to ensure their goals are achieved
- Proficient in Shiftcare and inputting funds.
- Proficient in SharePoint and major Microsoft Apps
- Rostering
- Ability to work as part of a team and/or on your own.
About You
- 2 Years Experience as Service Coordinator
- Experience in NDIS, disability services, and ideally community services.
- Strong understanding of NDIS guidelines and processes.
- Excellent communication and organisational skills.
- Ability to manage multiple participants and priorities
- Relevant qualification (e.g. Cert IV Disability preferred, Management, Community Services)

- Blue Card (Working with Children Check)
- Current driver’s licence *Police check
