About the Role
We are seeking a motivated Allied Health Assistant to join our HARP team within Home First, Ambulatory & Complex Care.
You will support multidisciplinary clinicians to deliver person-centred care to people with chronic and complex conditions, helping to improve health outcomes and reduce avoidable hospital presentations.
Key Accountabilities
Assist with exercise programs and functional interventions
Support equipment provision and client education
Assist clients to engage with appointments and self-management
Provide administrative support to care coordination
Work across home, community, and outpatient settings
About You
You will be a flexible, team-focused Allied Health Assistant who brings strong communication skills and a commitment to high-quality care. Together with:
Certificate IV in Allied Health Assistance (or equivalent)
Strong interpersonal and teamwork skills
Ability to work in a multidisciplinary environment
Competence with IT systems
Current driver’s licence; Working with Children Check, Police Check & immunisation compliance
Commitment to the Melbourne Way (People First, Lead with Kindness, Excellence Together)
Desirable
Experience in community or home-based care
Experience working with chronic disease, older people or complex needs
