The Care Coordinator provides leadership, coaching, and mentoring to nursing and personal care staff. They ensure the delivery of holistic, person-centered, and effective care that aligns with the "Better for Life" model. Additionally, the role supports the Care Manager in daily operations and staff management, while applying clinical judgement to maintain up-to-date service plans
Key Responsibilities
Health Promoting Services: Monitor quality assurance systems, conduct resident assessments, ensure timely medication administration, and maintain accurate care plans.
Person-Centered Care: Act as an advocate for residents, ensuring their goals and preferences are respected, risks are managed, and care is tailored to their social, cultural, and medical needs.
Governance & Leadership: Coordinate staff onboarding, provide performance feedback, facilitate appraisals, and monitor compliance with policies and procedures.
WHS Obligations: Ensure a safe environment by implementing WHS policies, managing hazards, and ensuring staff comply with safe work instructions.

We are looking for people who will be the difference in the lives of our residents and clients. To be successful in this role, you will have:
Qualifications: Bachelor of Nursing with current unrestricted AHPRA registration as an Registered Nurse and a current First Aid Certificate (HLTAID011) & CPR Certificate (HLTAID009)
Experience: Previous experience as a Registered Nurse with leadership responsibilities in an aged care setting.
Skills & Knowledge: Proficiency in Google Workspace and healthcare software, strong clinical management, time management, and problem-solving skills. Deep knowledge of Aged Care Standards, dementia care, and AN-ACC funding is essential.
