As Nurse Practitioner - Frail Older Persons, you'll play a vital role in improving care for older people with complex needs. From leading advanced clinical decision-making to shaping integrated care pathways, you'll work alongside a collaborative team to deliver meaningful, patient-centred outcomes across the service.
Every day, you'll contribute by:
Delivering advanced assessment, diagnosis and treatment that improves outcomes for frail older patients
Working collaboratively with multidisciplinary teams to coordinate care and support informed patient decisions
Leading and influencing best practice through clinical governance, quality improvement and innovation
Mentoring and guiding colleagues while strengthening systems that support safe, effective care

Here's what will help you thrive in this role:
Endorsement (or eligibility) as a Nurse Practitioner with current registration
Master's degree qualification relevant to Nurse Practitioner practice
Advanced clinical expertise in managing acute and chronic conditions in older people
Strong leadership, collaboration and critical thinking skills in multidisciplinary settings
You'll stand out from the crowd if you have experience working in geriatric or community-based models of care and/or a passion for driving service innovation and improvement.
Most importantly, we're looking for someone committed to enhancing quality of life for older people and shaping care that is compassionate, safe and forward-thinking.
