Role description
Inglewood and Districts Health is seeking a motivated and experienced Transition Care Program (TCP) Coordinator to join our team. This role is responsible for delivering high-quality assessment, care planning, and case management to eligible clients within both bed-based and home-based Transition Care services.
As the TCP Coordinator, you will play a key role in supporting people through their restorative journey following hospitalisation, helping them regain independence and transition safely to longer-term care arrangements.
Flexibility in hours within reason can be negotiated by the successful applicant
Key Responsibilities
- Provide comprehensive assessment, support, care planning, and case management for TCP clients and their carers
- Coordinate care across multidisciplinary teams and community services
- Develop strong collaborative relationships across Inglewood and Districts Health and external providers
- Ensure timely progression of clients through the TCP program
- Support clients to optimise their functional capacity and achieve their goals
- Facilitate discharge planning and access to appropriate long-term care services
- Act as the central point of contact for clients, families, and care providers
- Maintain accurate documentation and compliance with program guidelines
About you
Essential:
- Relevant qualification in Nursing, or Allied Health,
- Demonstrated experience in case management or care coordination
- Strong communication and interpersonal skills
- Ability to work collaboratively within a multidisciplinary team
- Knowledge of aged care or transition care services
