The Pathways to Community Living Initiative (PCLI) is a coordinated statewide approach to supporting people with severe, persistent and enduring mental illness who are “at risk” of a protracted hospital admission, are experiencing a protracted admission and/or have complex barriers to discharge and successful community living. PCLI supports consumers to transition from hospital and establish and sustain meaningful lives in the community through individualised, recovery-oriented and least restrictive approaches. Illawarra Shoalhaven Local Health District is committed to building a skilled and responsive workforce that supports consumers with severe and persistent mental illness (SPMI) and complex needs throughout their recovery journey.
The PCLI Senior Clinician works across multiple sites and service settings, providing advanced clinical assessment, intervention, consultation and care coordination for consumers referred to the PCLI pathway. The position has a particular focus on consumers with complex and co-occurring needs, which may include intellectual disability or cognitive impairment, alcohol and other drug use, physical health conditions, psychosocial disability, behavioural complexity, trauma, functional impairment and significant social or environmental barriers to discharge.
The position will work collaboratively with consumers, carers and families, treating teams and government and non-government partners to identify and address clinical, functional and systemic barriers to discharge and community living. The position will lead and contribute to practice improvement through quality improvement, service development, education and training, evaluation, research and other activities that strengthen outcomes for consumers with complex needs.

Shellharbour Hospital is a busy acute care facility which provides emergency services, elective surgical services, general medicine, antenatal and primary family health care, satellite renal dialysis, and adolescent and adult mental health services on site. It's an accredited healthcare facility with the Australian Council on Health Care Standards (ACHS) and is recognised as providing a high standard of care to the community.

Applicable vaccination requirements for a Category A role- Working With Children Check (WWCC)
- Current AHPRA Registration or Social Work degree and eligibility for the Australian Association of Social Workers (AASW)
- Registered Nurses must hold relevant post-registration qualification and at least 3 years' experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate
- Valid NSW Drivers Licence
