About the Role
As the NDIS Lead, you will provide strategic leadership across CALHN to shape how NDIS hospital pathways work in practice. You will partner with leaders, clinicians and external partners to deliver timely, consistent and high-quality NDIS hospital pathway processes that support safe and connected care progression for consumers across all our hospital settings, and support optimal transition back into the community.
This role leads the design, delivery and monitoring of standardised NDIS hospital pathways, provides expert advice for complex discharge planning, and builds capability through education and consultancy. You will work closely with inpatient teams and support outreach to sub-acute and community services, while maintaining strong partnerships with NDIA, Department for Health and Wellbeing and NDIS providers to improve flow, reduce delays and support smooth transitions of care.
Qualifications
AHP-4 For registered professions - An appropriate degree level qualification or equivalent which entitles general registration with the relevant Registration Board of Australia and Australian Health Practitioner’s Regulation Authority (AHPRA).
For Self-regulated professions - An appropriate degree level qualification or equivalent which entitles which gives eligibility for full membership of the professional body.
RN-4 Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.
