About the role
LAERS is seeking a passionate and motivated Support Coordinator to join our growing team. This role is ideal for someone who enjoys working closely with participants, families, carers, and service providers to help individuals achieve their NDIS goals and build greater independence. The Support Coordinator will provide person centred support coordination services, assist participants to navigate through NDIS, connect with appropriate supports and services, and promote capacity building through strong communication, advocacy, and community engagement. This role suits someone who can work both independently and collaboratively while maintaining professionalism, empathy, and strong organisational skills.

Main Duties and Responsibilities
· Coordinate and manage participant supports in line with NDIS plans and participant goals.
· Assist participants to understand their NDIS plans, funding, and available supports.
· Build participant capacity to independently manage supports and navigate services.
· Connect participants with community, mainstream, allied health, and specialist services.
· Monitor participant progress, budgets, and service delivery outcomes.
· Maintain accurate records, case notes, reports, and documentation.
· Liaise with participants, families, carers, support workers, providers, and external stakeholders.
· Advocate for participants and support using informed decision-making.
· Support participants to explore meaningful life opportunities, social inclusion, and independence.
· Ensure all work complies with NDIS Practice Standards, organisational policies, and relevant legislation.
· Participate in team meetings, supervision, and ongoing professional development.
· Escalate concerns, risks, incidents, or safeguarding matters appropriately.
· Always maintain confidentiality and professionalism inline with NDIS code of conduct and LAERS policies.
· Provide excellent customer service and maintain positive stakeholder relationships.
· Assist in achieving billable targets and effective time management requirements.
Experience and Qualifications
Essential Requirements:
· Current First Aid and CPR Qualifications
· National Police Records Check
· Working With Children Check (WWCC)
· NDIS Worker Screening Check
· Victorian Driver’s Licence and access to a reliable vehicle
· Strong written and verbal communication skills
· Ability to work independently and as part of a team
· Strong organisational, time management, and problem-solving skills
· Ability to build professional relationships with participants, families, and stakeholders
· Experience within Support Coordination, Case Management, Disability Services, Community Services, or related fields
· Knowledge and understanding of the NDIS and relevant legislation
· Tertiary qualifications in Community Services, Disability, Social Work, Welfare, or related disciplines
· Experience managing participant budgets and service agreements

Pay: $55,000.00 – $80,000.00 per year
