Position: Community & Personal Support Worker
We are currently seeking a passionate, experienced, and motivated Community & Personal Support Worker to join our team in Harris Park, NSW.
The successful candidate will play a vital role in supporting NDIS participants through case management, community engagement, daily living assistance, and capacity-building programs.
Key ResponsibilitiesClient Assessment & Planning
- Conduct intake interviews with NDIS participants and families to assess individual needs, goals, and barriers.
- Develop, implement, and review Individual Support Plans (ISPs) aligned with participant goals and NDIS funding.
- Collaborate with participants to create programs that support independence, education, employment readiness, and community participation.
Program Implementation & Support Delivery
- Provide direct support including personal care, daily living assistance, community access, psychosocial support, and capacity building.
- Facilitate workshops and activities focused on life skills, employment readiness, and social inclusion.
- Deliver support services within Supported Independent Living (SIL), respite, and community-based settings.
Monitoring, Reporting & Compliance
- Maintain accurate progress notes, participant records, and service reports in compliance with NDIS Quality and Safeguards Commission standards.
- Monitor participant progress and adjust support strategies where required.
- Assist with NDIS plan reviews and funding-related documentation.
Referral & Community Engagement
- Connect participants with specialist providers including therapists, housing services, and training organisations.
- Build strong relationships with local councils, advocacy groups, and community organisations.
- Represent COLLABORATIVE SUPPORT NETWORK PTY LTD at community events and disability support initiatives.
Family & Participant Support
- Provide guidance, emotional support, and respite assistance to families and carers.
- Support families in understanding NDIS processes, entitlements, and available community resources.
- Assist participants in overcoming social, emotional, and financial challenges.
Specialised Support (Where Required)
- Support participants involved with corrective or justice services in collaboration with relevant agencies.
- Deliver culturally appropriate and inclusive support services for participants from diverse backgrounds.
Skills, Experience & QualificationsEssential Requirements
- Diploma or higher qualification in Community Services, Social Work, Human Services, Welfare, or a related field.
- Minimum 2–3 years of experience in the disability, welfare, aged care, or community services sector.
- Strong understanding of the NDIS framework, participant rights, and safeguarding standards.
- Excellent communication and interpersonal skills with the ability to engage effectively with people from diverse cultural backgrounds.
- Demonstrated experience in case management, participant support planning, and report writing.
- Current National Police Check, Working with Children Check, and valid Driver’s Licence.
Desirable
- Previous experience within an NDIS-registered organisation.
- Training in Mental Health First Aid, Positive Behaviour Support, Disability Care, or related fields.
- Multilingual skills highly regarded to support our diverse community base.
