Based at Mona Vale Hospital, this exciting new Clinical Nurse Specialist Grade 2 (CNS2) position will serve as the senior clinician delivering both direct and virtual care to Hospital in the Home (HITH) patients referred through our Aged Care Outreach Services. Working predominantly with patients aged 70 years and over, you will collaborate closely with senior clinicians and geriatricians to provide care in patients' homes and residential aged care facilities, helping to prevent unnecessary emergency department presentations and avoidable hospital admissions.

What you'll be doing
As a Hospital in the Home CNS2 you will be assessing and treating acute patients referred to HITH from our Aged Care Outreach Service teams. Liaising directly with the CNC's and MDT, including ACOS gerontologists you will supporting these patients to remain in their home (often an aged care facility). Your expert clinical care will minimise the stress that elderly patients (and their families) experience when transferred to hospital unnecessarily.

Candidates will need to meet the following criteria:
- Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
- Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
- Demonstrated skills in computer use and applications such as Windows, word processing and database.
Demonstrated ability to work individually and as part of a multidisciplinary team.
