Nurse Next Door is a certified Great Place to Work and is on the lookout for an experienced Community Care Coordinator for aged care and disability clients in their homes.
This role is located in our Hawthorn office, our Headquarters for Nurse Next Door Australia. The role requires daily travel to clients homes and includes the use of our iconic Pink Car!
Due to the travel demands of this role, we require you to be located within the vicinity of the Hawthorn area.
Your Next Adventure
Reporting to our Clinical Operations Manager, your role will be managing your own client portfolio of both aged and disability clients. This includes but will not be limited to the following:
- Working one on one with clients and their loved ones, focusing on their individual nursing needs and happiness
- Leading a small team or nurses and caregivers assigned to your clients & be responsible for the development & review of Care Plans
- Competent in basic & complex would management
- Competent in catheter insertion -SPC and IDC (both female & male)
- Maintain meticulous care documentation records
- Continuous quality improvement

Mandatory Requirements
- Registered Nurse (AHPRA Registration required) with no conditions or limitations imposed on practice - minimum 5 years experience
- Experience in either a Home Care setting or Aged Care facility, leading a small team of Caregivers & Nurses is highly regarded
- Minimum 5+ years nursing experience in home care or aged care facility settings required
- Current Victorian Drivers License
- NDIS Workers Screening Check, or willing to obtain clearance
- NDIS Workers Screening Check must be completed before starting employment
- Working With Children Check - employee status , or willing to obtain
- Computer literate
- Must live within the vicinity of the Hawthorn area
Pay: $120,000.00 – $140,000.00 per year
