The Role
An opportunity has become available for a Home Care Coordinator to join Gold Coast Health (GCH) where the focus of our 13 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
Undertake effective case planning and management through client consultation, assessment and review
Supervise a team of Home Care Workers and actively promote a positive workplace culture
Participate in policy and procedure initiatives and continuing quality improvement activities
Coordinate recruitment, rostering, payroll, training, and support for Home Care Workers
About you
To be successful in this position you will require:
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Driver Licence
Declaration of serious discipline history must be disclosed
Aged Care Check will be required for new employees

Salary
Permanent part time and temporary part time (one position, up to six months with possibility of extension, 45.60 hrs per fortnight)
Base salary ranging from $49.31 - $ 51.98 per hour
About Us
The Gold Coast Home Care and Allied Health service is a Commonwealth Home Support Program funded service delivered by Gold Coast Health and funded by the Commonwealth. This community-based program employs approximately 120 staff who deliver in home care to people living across the Gold Coast Health region.
The Home Care and Allied Health service is an entry level service providing a range of programs within the Commonwealth Home support Program Guidelines including Domestic Assistance, Personal Care, Social support, Group social support, in home respite, and specialised clinical services in Allied Health, including individual and group allied health.
