About the Role
You will deliver tailored occupational therapy services to older Australians under Support At Home (SAH) and Commonwealth Home Support Program (CHSP) frameworks. Your responsibilities will be aligned with your level of experience and may include:
- Conducting holistic occupational therapy assessments.
- Developing and implementing individualised care plans.
- Prescribing and coordinating assistive equipment and home modifications.
- Liaising with clients, families, GPs, and allied health professionals to ensure integrated care delivery.
- Empowering clients to manage their own health through sustainable capacity-building strategies.
- Maintaining timely and accurate documentation including care plans, progress notes, and reports.

To be considered, you will need:
- A bachelor’s degree in occupational therapy (or completion by the time of appointment).
- Current or eligible AHPRA registration.

- For new graduates: a willingness to learn and contribute to a multidisciplinary team.
- For experienced candidates: confidence in managing complex cases and mentoring others.
