About the Opportunity
As a Community Speech Pathologist with the Transitional Aged Care Program (TACP), you will provide high-quality, evidence-informed assessment and intervention to older persons living in the community with a diverse range of health conditions, including neurological conditions, age related functional difficulties, progressive conditions and chronic diseases such as Diabetes, Respiratory, Heart Failure and Cardiac diseases.
You will deliver services in clients' homes and community health centres, helping people achieve meaningful communication and swallowing outcomes that enhance their quality of life. In this role you will:
- Deliver evidence-based speech pathology assessment and intervention to adults in community settings.
- Empower clients living with neurological conditions, chronic disease and age-related challenges to achieve meaningful health outcomes
- Collaborate with a supportive multidisciplinary team to provide high-quality, person-centred care
- Contribute to service innovation, quality improvement and research while growing your professional expertise.

About You
We are looking for someone who has:
- Tertiary qualifications in Speech Pathology and eligibility for membership with Speech Pathology Australia.
- Current registration (Certified Practising Speech Pathologist or eligibility to obtain) and a commitment to evidence-based practice.
- Experience delivering high-quality assessment and intervention for adults with neurological conditions, chronic disease and age-related communication and swallowing needs.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families and multidisciplinary teams.
- A collaborative, compassionate and proactive approach, with a commitment to continuous learning, quality improvement and delivering person-centred care.
