About the Role
We are seeking a motivated and compassionate Disability Support Worker to provide person-centred support to individuals with disability. This role involves assessing client needs, developing support programs, and providing ongoing assistance to individuals and families. This position requires strong interpersonal skills and commitment to improving client outcomes. You will play a key role in supporting clients to achieve their personal goals.
Main Duties:
- Person Centred Support
- Assist clients in accessing community, health and social services.
- Monitor client progress and maintain accurate records and reports.
- Consulting with the client, relevant stakeholders, and developing a plan and implementing a program to address clients need and support goals.
- Arrange meetings with teams, families and stakeholders to provide additional support to the organisation and clients.
- Prepare reports, submissions, and documentation for funding bodies and government agencies.
Skills and Experience:
- Qualification in Community Services, Disability, or a related field (Certificate III or above)
- Strong understanding of person-centred approaches and community-based support.
- Excellent communication and interpersonal skills.
- Current NDIS Worker Screening Check
- Current and clear Criminal Record Check
- Understanding of privacy, confidentiality and ethical practice standards.
- Current Driver's Licence.
