Harmonious Community Care is looking for compassionate, reliable and dedicated Support Workers to join our growing team in Darwin, Northern Territory.

Female Support Workers are preferred for this position.
About the Role
As a Support Worker with Harmonious Community Care, you will provide person-centred support to NDIS participants in their homes and within the community.
You will work closely with participants, families and other members of their support team to ensure participants receive safe, respectful and high-quality support.
Key Responsibilities
Your duties may include:
- Providing personal care and daily living support, including assistance with showering, dressing, grooming and other personal-care tasks where required
- Supporting participants with meal preparation, cooking and household tasks
- Assisting with medication prompts and medication-related support, in accordance with participant plans and organisational procedures
- Supporting participants to attend appointments, therapy sessions and other commitments
- Providing transport and support to access the community, social activities, shopping and recreational activities
- Supporting participants to develop and maintain independence and daily living skills
- Encouraging participation in activities that align with the participant's interests, goals and NDIS plan
- Providing support with community participation and social inclusion
- Supporting participants to maintain routines and engage in meaningful activities
- Following individualised support plans, risk assessments and behaviour support strategies where applicable
- Recognising and appropriately responding to changes in a participant's presentation, wellbeing or support needs
- Maintaining accurate and timely progress notes, shift notes and incident reports
- Communicating professionally with families, Support Coordinators, allied health professionals and other members of the participant's support team
- Maintaining appropriate professional boundaries, confidentiality and participant privacy
- Following all workplace health and safety, safeguarding and organisational policies and procedures
- Promoting participant choice, dignity, rights, privacy and independence at all times
About You

- Able to maintain accurate documentation and communicate concerns appropriately
Essential Requirements
Applicants must have:
- Previous experience working as a Support Worker, Disability Support Worker, Aged Care Worker, Personal Care Worker or in a similar role is highly desirable
- NDIS Worker Screening Check
- Working with Children Check, where applicable to the role
- Current First Aid Certificate
- Current CPR Certificate
- NDIS Worker Orientation Module – Quality, Safety and You
- Full Australian working rights
- A valid Australian driver's licence
- Access to a reliable, registered and comprehensively insured vehicle, where driving participants is required
- Good written and verbal communication skills
- Ability to complete required documentation and progress notes
- Commitment to maintaining participant confidentiality and privacy
- Relevant Qualification such as Certificate III/IV in Individual Support, Disability, Community Services, Mental Health or a related field

Availability
