About the Role:
As a Case Manager at Bliss Nursing, you'll play a vital role in coordinating care for our clients, helping them navigate aged care systems with confidence. Your cultural insight and language skills will be key in building trust and ensuring personalized, effective support.
Key Responsibilities:
- Manage client caseloads with empathy and professionalism
- Conduct assessments and develop tailored care plans
- Support clients and families in navigating aged care systems
- Liaise with service providers, healthcare professionals, and community networks
- Maintain accurate documentation and compliance with regulatory standards
Requirements:
- Fluent in Arabic(preffered) and English.
- Proven experience as a Case Manager, preferably in aged care
- Strong knowledge of aged care standards,home care packages, support at home, and client-centered care
- Excellent communication and interpersonal skills
- Relevant qualifications in community services, social work, or nursing
