As a Social Worker, you will be responsible for the assessment, intervention and management of patients and their families requiring Renal Services. Working as part of a multidisciplinary health team, you will identify psychosocial needs and deliver a range of social work interventions that promote continuity of care, patient safety and optimal outcomes. This advanced practice role requires strong clinical reasoning, professional judgement and the ability to manage novel, complex and critical situations with a high degree of autonomy and minimal direct clinical supervision.
