Baldwin Care Collective is expanding, and we’re seeking a warm, organised, and people‑focused Care Coordinator to support our frontline team. This part‑time middle‑management role is ideal for someone who enjoys a blend of leadership, hands‑on support, and meaningful connection with participants.
About the Role
As our Care Coordinator, you’ll oversee and support our field staff, including Support Workers and Cleaning Staff. You’ll help ensure services are delivered safely, consistently, and in alignment with our values. You’ll work closely with the Operations Manager to maintain quality, respond to day‑to‑day needs, and strengthen participant outcomes.
This role includes field‑based work, such as participant visits, staff check‑ins, and service reviews, and leave coverage. Perfect for someone who likes to stay connected to the people we support.
Key Responsibilities
- Provide leadership, coaching, and support to Support Workers and Cleaning Staff
- Oversee daily service delivery and participant experience
- Conduct field visits, service reviews, and participant check‑ins
- Manage scheduling and roster coverage, including filling gaps, responding to changes, and ensuring continuity of care
- Facilitate training, onboarding, and skill development for field staff
- Support performance management, including feedback, documentation, and improvement plans
- Monitor compliance with NDIS Practice Standards, safety requirements, and organisational policies
- Assist with incident follow‑up, reporting, and continuous improvement
- Build strong relationships with participants, families, and providers
What We’re Looking For
- Experience in disability, aged care, community services, or similar frontline environments
- Strong communication and relationship‑building skills
- Confidence in decision‑making and managing day‑to‑day operational challenges
- Ability to support and guide staff with clarity, kindness, and accountability
- Understanding of NDIS practice standards
