Prom Country Aged Care (PCAC) is a community-based residential aged care service located in Foster, Victoria. It is registered as an incorporated association and its voluntary Board of Management is drawn from the organisation’s membership base, which represents a cross-section of the community. PCAC provides ageing in place residential care services as well as short-term respite care for its local community.
A new Model of Care is being rolled out, and requires a Project Lead to manage the roll out.
The primary reason for the proposed Model of Care is to improve the quality, safety and overall well-being of residents.
Key features of the new model
- Increased direct care staffing at peak times
- Redesign of roles to support hands-on resident care
- Better alignment of staffing with high-risk and high-demand periods
- Continued delivery of all existing services
Key Responsibilities
- Workforce Redesign: Assistance in redesigning existing roles within the care staff and catering team. This will include planning and management of upskilling existing staff with targeted training to ensure that staff time, skills, and roles are better aligned to resident needs and peak care demands
- Set up and Implementation of training pathways. Ensuring that no staff member is expected to undertake new duties without appropriate preparation, training and ongoing support
- Set up of systems and processes for Management to refer to both during the rollout of the new MOC and ongoing
- Liaising with staff, facilitating and resolving questions, and addressing barriers that may arise from the new Model of Care and the changed hours and work requirements
- Facilitate communication with Union representatives regarding the changes to employee contracts and hours
- Assist and coordinate the rollout and collection of contract variations
- Introducing change in a transparent and supportive way for residents, maintaining a high-quality care for them

- Demonstrated advanced IT literacy including proficiency in Microsoft Office applications
- Tertiary Business (HR) or Project Management qualification, or demonstrated 5 years experience
- Previous experience in a management or project lead role
- All PCAC appointments are subject to a satisfactory police check with no disclosures
Pay: $45.00 – $50.00 per hour
