Full-Time Support Coordinator / Psychosocial Recovery Coach
Centre of Hope is looking for a passionate and experienced Support Coordinator / Psychosocial Recovery Coach to join our growing team.
This is a permanent full-time position working from home, with regular travel to meet participants across the community.
Salary: $73,000 per annum + Superannuation
About the role
As a Support Coordinator and Psychosocial Recovery Coach, you will work closely with NDIS participants to help them achieve their goals, build their capacity, and connect with the supports they need. You will be supported by an experienced Team Leader and work within a collaborative and supportive team environment.

Key responsibilities
- Provide NDIS Support Coordination and Psychosocial Recovery Coaching.
- Build strong relationships with participants, families, guardians and service providers.
- Coordinate services and support participants to achieve their NDIS goals.
- Complete case notes, reports and other administrative tasks within required timeframes.
- Maintain accurate records using Microsoft Office and other online systems.
- Meet service delivery KPIs while maintaining a high standard of participant support.
- Work collaboratively with the Team Leader and the wider team.
Essential requirements
- Previous experience in the disability sector.
- Experience as a Support Coordinator and/or Psychosocial Recovery Coach is highly regarded.
- Certificate IV or higher in Disability, Mental Health, Community Services or a related field.
- Excellent communication and relationship-building skills.
- Strong organisational and time management skills.
- Good computer skills, including Microsoft Office and online systems.
- A current driver's licence.
- Reliable motor vehicle with comprehensive insurance.
- Willingness to travel to participant appointments.
- Current NDIS Worker Screening Check.
- Working With Children Check.
- National Police Check.
- Current First Aid and CPR (or willingness to obtain).
- Any other relevant compliance requirements to work within the NDIS.
