About the role
Join the Social Work team at The Royal Melbourne Hospital and make a meaningful difference to patients living with complex renal conditions.
As a Grade 3 Social Worker, you’ll provide specialist psychosocial care across our Kidney Care Service, supporting patients and families through chronic kidney disease, dialysis and transplantation. You’ll also provide clinical leadership, supervision and mentoring while contributing to service improvement and high-quality, patient-centred care.
Key Accountabilities
Provide psychosocial assessment, counselling and practical support to renal patients and their families/carers.
Support patients through chronic kidney disease, dialysis, transplantation and treatment decisions.
Collaborate with the renal multidisciplinary team and community services to achieve positive patient outcomes.
Provide supervision, mentoring and clinical support to junior Social Workers and students.
Participate in regional renal clinics and outreach services.
Contribute to service improvement, quality initiatives and best-practice social work.
Help deliver high-quality, compassionate and patient-centred care.
About You
You are a qualified Social Worker with AASW membership or eligibility, bringing:
Clinical experience supporting patients with chronic and complex health needs.
Strong leadership, supervision, mentoring and multidisciplinary collaboration skills.
Excellent communication, counselling and advocacy skills, with the ability to work autonomously and proactively.
A commitment to high-quality, consumer-focused care for patients, families and carers.
Experience in renal, chronic disease, outpatient or hospital settings will be highly regarded.
