About the Role:
Clare Holland House provides Specialist Palliative Care Services throughout the ACT including Home-based, Inpatient, Outpatient services along with outreach to Residential Aged Care Facilities and North Canberra Hospital. The Inpatient Hospice, situated on the shores of Lake Burly Griffin in Barton, provides admission for End-of-life care, symptom management and optimisation along with carer respite for patients experiencing a range of life-limiting illness.
The PEACE team’s Palliative Care Nurse Practitioners deliver compassionate, expert consultative care to residents with life-limiting illnesses in residential aged care facilities throughout the ACT. We are dedicated to supporting each individual’s unique palliative care needs, enhancing comfort, dignity, and quality of life.
