ABOUT THE ROLE
Pro Care Assist Support Services Pty Ltd is a registered NDIS provider delivering person-centred disability and community support services across Australia.
We are seeking an experienced and motivated NDIS Client Services Coordinator / Case Manager to support participants in achieving their goals, accessing services, and enhancing their independence and community participation.
The role is responsible for coordinating participant supports, facilitating service access, maintaining stakeholder relationships, monitoring participant outcomes, and ensuring services are delivered in accordance with participant goals, NDIS Practice Standards, and organisational policies.
The position combines client service coordination, case management, community engagement, advocacy, and support planning to ensure participants receive high-quality services that promote wellbeing, inclusion, and positive life outcomes.
KEY RESPONSIBILITIES
Client Relationship Management
- Build and maintain positive relationships with participants, families, carers, advocates, and support networks.
- Act as a primary point of contact for participant enquiries, support requests, and service-related matters.
- Monitor participant wellbeing and respond appropriately to changing needs and circumstances.
- Support participants to achieve their goals through person-centred and strengths-based approaches.
Service Coordination & Case Management
- Conduct participant needs assessments and identify barriers to accessing services and community participation.
- Coordinate and monitor the delivery of NDIS-funded supports to ensure participant goals and outcomes are achieved.
- Develop, implement, and review participant support plans and service arrangements.
- Facilitate referrals to community services, healthcare providers, education services, employment supports, and other relevant programs.
- Liaise with support workers, allied health professionals, support coordinators, and external service providers to ensure effective service delivery.
Community Participation & Advocacy
- Support participants to access community activities, programs, education, employment, and social opportunities.
- Advocate for participant rights, choice, control, and access to appropriate supports.
- Promote independence, social inclusion, and community engagement.
- Assist participants to overcome barriers affecting service access and community participation.
Compliance & Quality Assurance
- Maintain accurate participant records, case notes, service documentation, and progress reports.
- Ensure compliance with NDIS Practice Standards, organisational policies, and privacy requirements.
- Support incident management, risk assessment, safeguarding, and quality assurance processes.
- Participate in continuous improvement activities and service reviews.
ESSENTIAL REQUIREMENTS
- Qualification in Community Services, Community Work, Human Services, Disability Services, Social Welfare, or a related field.
- Demonstrated experience within disability services, community services, NDIS, case management, or client service coordination.
- Knowledge of NDIS Practice Standards, participant safeguarding, and service delivery requirements.
- Strong stakeholder engagement, communication, advocacy, and relationship management skills.
- Experience supporting vulnerable individuals and coordinating community-based services.
- Current NDIS Worker Screening Check.
- Current Working With Children Check (where applicable).
- Current First Aid and CPR Certification.
- Current Australian Driver Licence.
Pay: $80,000.00 – $85,000.00 per year
