CARE COORDINATOR – DISABILITY SERVICES
Sovereign Lives Victoria is seeking an experienced, organised and participant-focused Care Coordinator to join our team.
Sovereign Lives Victoria provides disability support services designed to promote participant choice, independence, safety and meaningful community participation. We are looking for a capable professional who can coordinate high-quality supports while maintaining strong relationships with participants, families, employees and external stakeholders.
About the position
The Care Coordinator will oversee the coordination and day-to-day delivery of participant supports. The position requires sound judgement, strong communication skills and the ability to manage operational priorities while maintaining compliance with NDIS requirements and organisational policies.
Key responsibilities include:
- Coordinating participant supports in accordance with individual goals, service agreements and approved support plans
- Maintaining regular communication with participants, families, nominees, support coordinators and relevant healthcare professionals
- Supporting the development, implementation and review of participant support plans
- Coordinating staff allocations and assisting with roster-related requirements
- Providing guidance and operational support to Disability Support Workers
- Monitoring the quality and consistency of service delivery
- Maintaining accurate participant records, progress information and compliance documentation
- Reviewing incidents, concerns, complaints and changes in participant needs and ensuring appropriate escalation
- Supporting participant transitions, appointments, service reviews and stakeholder meetings
- Ensuring services are delivered in accordance with the NDIS Practice Standards, NDIS Code of Conduct and organisational procedures
- Contributing to staff training, supervision, performance monitoring and continuous improvement activities
About you
The successful applicant will demonstrate:
- Relevant experience within the disability, community-services or healthcare sector
- Previous experience in care coordination, service coordination or a similar position
- Strong knowledge of person-centred practice and participant choice and control
- An understanding of NDIS requirements, incident management, safeguarding and service documentation
- Excellent written and verbal communication skills
- Strong organisation, time-management and problem-solving abilities
- The capacity to manage competing priorities and respond professionally to urgent matters
- The ability to work collaboratively with participants, families, employees and multidisciplinary stakeholders
- Professional judgement, accountability and attention to detail
- Confidence using electronic documentation and workforce-management systems
Applicants must hold relevant qualifications and maintain all required employment checks, including an NDIS Worker Screening Check, Working with Children Check, National Police Check and a valid driver’s licence.

Pay: $35.00 – $40.00 per day
