Registered Nurse required to attend to clients around the Perth North Metropolitan region. This is an interesting and varied position and would suit someone with experience as a Community Nurse.
Do you meet the following criteria:
- Experience in case management in Aged Care or community services sector, managing client budgets and organising Allied Health and services.
- Clinical experience with medication management, wound care, catheters etc.
- Knowledge of Home Care Packages, Community Home Support Programme and Aged Care Quality Standards.
- Training and Assessment certificate (preferred)
- Knowledge of NDIS will be an advantage
- Attend and support Franchise accreditation visits and contribute to responses and communications with the Commission.
- Provide feedback regarding issues with systems to the Franchisor Clinical Governance team.
- Ensure Franchisee compliance in mandatory quality indicators, auditing, reporting, and reviewing of clinical incidents and provide weekly reports to the Franchisor Compliance Team
- Participate in nominated Focus groups/committees (internal and external)
- AHPRA registration.
- National Police Clearance.
- The right to work in Australia.
- The ability to work as part of a team and autonomously.
Duties will include but not limited to:
- Conducting Comprehensive Assessments, co-creating Care Plans for clients and conducting 6 monthly and 12 monthly reviews when required.
- Ensure client services are resourced efficiently and delivered according to legislative and best practice guidelines.
- Maintain communication with client and /or client representatives and other bodies as necessary.
- Participate in risk identification, assessment and control.
