Cessible Healthcare is seeking a compassionate, reliable Registered Nurse (RN) to provide high-quality community nursing support to people in their homes, including older adults and people living with disability. This role focuses on maintaining independence, improving wellbeing, and preventing avoidable hospital admissions through person-centred care.
About the role
As a Community RN, you will deliver clinical care in the home and community, support safe medication management, provide wound and chronic disease care (including diabetes support), and help train and guide support staff. You will also complete assessments, coordinate with allied health and other providers, and escalate concerns appropriately.
Roles and responsibilities:
Clinical Care and Assessment:
- Complete holistic nursing assessments (physical, psychosocial, functional and clinical risk)
- Develop and review individualised care plans aligned to client goals and funding (NDIS/HCP where relevant)
- Provide ongoing clinical monitoring, follow-ups and post-hospital visits
- Recognise deterioration early and escalate appropriately to families, GPs, services, or emergency supports
Medication management and clinical interventions:
- Support safe medication administration, documentation and monitoring
- Provide clinical interventions such as:
- Wound care and wound monitoring
- Diabetes assessment and management support (BGL monitoring, education, escalation pathways)
- Continence and skin integrity support
- Basic clinical observations and health screening as required
- Monitor response to treatments and update care plans accordingly
Education and Support
- Educate clients/participants and their families or carers about health conditions, treatment plans, and self-care strategies to support informed decision-making and confidence at home
- Advocate for clients’ rights, choices, and preferences, ensuring they are actively involved in their care and goal planning
- Provide clinical advice and support to support workers and care staff
- Deliver education and skills coaching within your scope (for example medication support processes, wound care principles, infection control)
- Delegate and monitor care tasks appropriately, ensuring safe practice, documentation, and escalation when required
Communication and coordination:
- Liaise with GPs, allied health, hospitals, families, and other service providers
- Participate in case conferences and contribute to coordinated care
- Complete clear clinical documentation, reports, and handovers
Safety, compliance and quality:
- Follow infection control and safety procedures in all environments
- Work in line with AHPRA/NMBA standards, professional boundaries, and ethical practice
- Support continuous improvement and quality care delivery
What we’re looking for:
- Strong clinical assessment skills and sound judgement
- Calm, professional approach when responding to change or clinical risk
- Excellent communication skills (written and verbal)
- A person-centred approach that respects dignity, choice, and independence
- Confidence working autonomously in the community and collaboratively within a broader care team
Essential requirements:
- Current AHPRA registration as a Registered Nurse
- Australian work rights
- Current driver’s licence and ability to travel for shifts
- Current Police Check
- Manual Handling certificate
- First Aid certificate with current CPR (or willingness to obtain promptly)
- Working With Children Check (if required for the role/client group)
Desirable (but not required):
- Community nursing experience, disability nursing, or aged care experience
- Knowledge of NDIS and/or Home Care Packages environment
- Additional training in wound care, diabetes care, continence, palliative care, dementia care, etc.
- Training and assessment qualification (Cert IV TAE) or experience mentoring staff
