What is the Housing Support Program at CatholicCare NT?
CatholicCare NT (CCNT) implements a case management approach to break the cycle of homelessness through empowering clients to effectively problem solve and manage their accommodation. CCNT uses an early intervention approach that supports individual clients to regain control of their circumstances including housing, financial, and social and community issues that are placing them at risk of homelessness.
Through assertive case management we provide clients with support to address the multifaceted challenges they face in maintaining their home. This support may be through assessment of life skills, role modelling, referral to specialist agencies as required, coordination of supports, and advocacy with internal and external agencies.

More on what you'll be doing as a Case Worker in Housing Support…
The Case Worker focuses on providing assertive case management, education and support to participants with a stepped model of care which ensure clients receive a targeted service according to need. This includes low, medium and high intensity support and case management.
You will deliver outreach case management support to secure housing through linking with local housing providers, coordinate support services to people in public housing to improve their capacity to obtain and retain housing; support transition out of public housing and provide tailored assistance, support and advocacy.
- You will solve problems using your knowledge, judgement and organisational skills and deliver outreach case management support which secures housing.

You will support immediate access to housing by using your networking and communicaton skills to link with local accommodation providers, public housing, social housing and the private rental market.

You will use your experience to make clear and detailed case notes, deliver and coordinate support services to people living in public housing to improve their capacity to obtain or retain housing

You will use your interpersonal skills, problem solving skills and compassion to promote social integration to enable clients to live independent lives

You will accept, prioritise and process referrals and develop strategies to address challenges to achieving social and emotional wellbeing

Your report writing skills will help you as you develop case plans which identify short, medium and long term case management goals.

You will report to the Team Leader Housing and Assertive Outreach in Tennant Creek

1) You are an Australian Citizen or have a valid visa with unlimited work rights in Australia and hold a current Driver's Licence.
2) Relevant Certificate and/or work experience to enable you to undertake the full scope of the role (Check the Role Description available on our website).
3) Strong interpersonal and communication skills including the ability to engage with people in person and in writing, and to manage complex situations, involving homelessness, poverty, mental health, family violence, alcohol, and other drug misuse.
4) Demonstrated ability to advocate for clients, maintain confidentiality and work effectively with stakeholders to meet the objectives of the Housing Support Program.
5) Demonstrated cultural competency, particularly in working with Indigenous people.

If you have resided in an overseas country for 12 months or more in the past 10 years, this position requires you to complete an International Criminal History check (ICHC) prior to your employment commencement date. The outcome of the initial screening check must be satisfactory.
