Allegiance Heart Home Care is seeking a reliable and experienced Physiotherapist to provide community-based physiotherapy support to aged care clients in their homes.
We are currently seeking a physiotherapist who can support an elderly client in the Weir Views/Melton South area following hospital admission and rehabilitation after a fall and fracture.
The client will require physiotherapy support to improve strength, conditioning, mobility, transfers, confidence and falls prevention.
This role is suitable for a physiotherapist who enjoys working with older people in the community and can provide safe, person-centred and goal-focused therapy.
About the role
You will provide physiotherapy services in the client’s home, including assessment, treatment planning, mobility and transfer support, strengthening exercises, balance work, falls prevention strategies, and practical recommendations to improve safety and independence at home.
The client may require ongoing weekly physiotherapy, with potential for additional shifts.
Key responsibilities
- Complete home-based physiotherapy assessment and treatment.
- Support mobility, strength, balance and conditioning.
- Provide falls prevention strategies and education.
- Assess transfer safety and recommend safe mobility techniques.
- Develop a simple home exercise program suitable for the client’s needs.
- Communicate clearly with the client, family and care team.
- Provide brief written progress notes and recommendations after sessions.
- Escalate any concerns relating to safety, mobility, pain, deterioration or equipment needs.
- Work in a person-centred manner that respects the client’s home, dignity and preferences.
Essential requirements
- Current AHPRA registration as a Physiotherapist.
- Experience working with older people, rehabilitation, falls prevention or community aged care.
- Current police check.
- Own vehicle and willingness to travel to client homes.
- Strong communication and documentation skills.
- Reliable, professional and compassionate approach.
Highly regarded
- Experience with post-hospital discharge rehabilitation.
- Experience with hip/femur fracture rehabilitation or post-operative mobility support.
- Experience working with clients who use walking aids or require transfer supervision.
