Aged Care Case Manager – Central Coast & Newcastle
Part-time | Work from home + local client visits | Support at Home
We are looking for a Case Manager who does not need to be managed to do the job well.
You will be trusted to take ownership of your caseload, stay on top of care plans, reviews, client contact, documentation and follow-up, and raise issues early. You will also be part of a supportive, experienced team that meets daily and is available when you need guidance or a second opinion.
This is a role for someone who enjoys autonomy, takes pride in being organised and accountable, and genuinely cares about helping older Australians live safely and independently at home and has managed clients before.
Autonomy here means ownership — not isolation. You will have clear expectations, daily team contact, ongoing training and accessible support, but you will not need someone reminding you to complete the basics of your role.
What you will own
· Your caseload. Manage your allocated aged care clients proactively and make sure nothing important is left waiting for someone else to notice.
· Care planning. Write, review, coordinate and oversee personalised care plans under the Support at Home model, including keeping Visual Care information current.
· Client contact and reviews. Maintain regular contact with clients and families, complete required reviews and support new clients through onboarding.
· Relationships. Build trusted, professional relationships with clients, families, support workers and other stakeholders.
· Documentation and follow-through. Keep notes and required records up to date, review Sentiment Dashboard information for your clients, and finalise incidents and actions promptly.
· Quality and outcomes. Make sure services align with the Aged Care Quality Standards and that clients receive safe, responsive and respectful support.
· Team contribution. Attend daily team meetings, communicate clearly, complete weekly training and ask for help early when a matter needs escalation or another perspective.
The person we are looking for
· You are self-starting. You can plan your day, prioritise competing work and follow tasks through without repeated prompting.
· You are accountable. If something belongs to your caseload, you take responsibility for progressing it and keeping the right people informed.
· You are organised. You are comfortable managing care plans, reviews, calls, notes, incidents and follow-ups across multiple clients.
· You are confident working from home. You have demonstrated experience working autonomously and staying productive without constant supervision.
· You are warm and professional. You listen well, communicate clearly and can build trust with older people, families and colleagues.
· You know aged care case management. Previous case management experience is essential; experience under Home Care Packages is welcome, and you will need to be ready to work within the Support at Home environment.
