Community Worker

Position Overview
We are seeking a compassionate, reliable and experienced Community Worker to join our growing team on a full-time, permanent basis.
The successful candidate will provide individualised support to people living with disability and other support needs, assisting clients to participate in community life, develop independence and achieve their personal goals. The role will involve assessing client needs, implementing support programs, assisting with daily living and community access, and coordinating with families, carers, service providers and other stakeholders.
The ideal candidate will have a sound understanding of person-centred support and demonstrated experience within disability, community services, social welfare or aged care environments. Experience supporting individuals with complex needs or mental health conditions will be highly regarded.
Key Responsibilities
Assess the needs of individuals and families and assist in planning, developing, implementing and reviewing appropriate support programs.
Provide practical and emotional support to clients to assist them with daily living, household tasks, personal activities and community participation.
Support clients to access health appointments, community activities, social events, recreational programs and other services relevant to their individual goals.
Assist individuals to develop and maintain daily living skills that promote independence, confidence and participation within the home and wider community.
Support clients in supported independent living and other individualised home and living arrangements in accordance with their care and support plans.
Interview clients, identify their support requirements and assist in responding to issues affecting their wellbeing, independence and access to services.
Liaise with families, carers, community groups, welfare agencies, government bodies and other service providers to coordinate appropriate support and service connections.
Assist clients to access appropriate community, health, housing, education, employment and other support resources.
Support individuals experiencing mental health concerns, social isolation, vulnerability or complex support needs and refer matters requiring specialist intervention.
Encourage and facilitate social inclusion, community engagement and participation in meaningful recreational and cultural activities.
Assist clients with goal setting, recovery planning, capacity building and development of skills required for greater independence.
Monitor client progress and maintain accurate case notes, support records, incident records and other documentation in accordance with organisational procedures and relevant NDIS requirements.
Report changes in client circumstances, health, behaviour, risks or support requirements to relevant supervisors and health or support professionals.
Participate in the review and evaluation of support programs and provide feedback to management regarding client outcomes and service delivery.
Provide assistance during respite, accommodation and community-based support activities as required.
Required Skills and Qualifications
Qualification and Employment
- A relevant AQF Associate Degree, Advanced Diploma or Diploma in Community Services, Social Work, Disability, Individual Support or a related field.
- OR
- In lieu of formal qualifications, a minimum of three (3) years of relevant full-time experience in a community worker or closely related community services role may be considered.
- A minimum of one (1) year of recent relevant full-time experience working in a similar role within the disability, community services or aged care sector.
Required Skills and Attributes
- Demonstrated experience assessing client needs and implementing individualised support programs in a disability, community services or aged care setting.
- Sound understanding of person-centred care principles and the rights, needs and support requirements of people living with disability.
- Experience supporting individuals with complex needs, mental health conditions, behavioural challenges or other vulnerabilities will be highly regarded.
- Demonstrated ability to support clients with daily living activities, community access, social participation and development of independent living skills.
- Ability to liaise effectively with families, carers, community organisations, welfare agencies, government bodies and other service providers.
- Strong interpersonal and communication skills with the ability to establish trust and rapport with clients from diverse backgrounds.
- Ability to maintain accurate client records, case notes, incident reports and support documentation in accordance with organisational and NDIS requirements.
- Strong organisational, problem-solving and time-management skills, with the ability to manage multiple client needs and competing priorities.
- Demonstrated ability to work independently and collaboratively while maintaining professional boundaries and confidentiality.
- Current First Aid and CPR certification.
- Valid NDIS Worker Screening Check and Working with Children Check, or willingness to obtain prior to commencement.
- Current Police Check.
- A valid driver's licence and access to a reliable vehicle.
- Ability to work flexible hours, including weekends, evenings and public holidays where required by client support arrangements.
