About the Role
Reporting to the Executive Leadership Team and working closely with clinical and operational leaders across Vitalis, you will combine direct clinical service delivery with broader service development initiatives.
You will support clients in their homes and via telehealth, helping them maintain mobility, independence, safety and quality of life. At the same time, you will contribute to service innovation, clinical excellence, referral growth and the continued expansion of our allied health offering.
This role is ideal for an experienced physiotherapist who enjoys meaningful client contact while also wanting to influence how high-quality healthcare-at-home services evolve and grow.
Key Responsibilities

Deliver evidence-based physiotherapy assessments and treatment plans in clients' homes and via telehealth.
- Support older people, post-acute clients, people living with disability and individuals with complex health needs.
- Collaborate closely with nurses, doctors, care managers and external healthcare providers to deliver coordinated multidisciplinary care.
- Contribute to clinical governance, quality improvement, service growth and the recruitment and development of future allied health team members.
- Develop strong referral and stakeholder relationships while identifying opportunities for service growth and expansion
Skills & Experience

Current AHPRA registration as a Physiotherapist.
- Significant post-graduate clinical experience in community health, aged care, rehabilitation, disability, hospital or complex care settings.
- Strong clinical reasoning, assessment and treatment planning skills.
- Demonstrated ability to work autonomously while contributing effectively to a multidisciplinary team.
- Excellent communication, relationship-building and stakeholder engagement skills.
Experience within Healthcare-at-Home, aged care, NDIS, home care or palliative care settings will be highly regarded but is not essential.
