What you’ll be doing
As a Senior Social Worker, you’ll play a pivotal role in supporting clients and families through their rehabilitation journey. You will provide advanced clinical expertise and leadership across the Acute Stroke Unit and Geriatric Medicine services. Working with a high degree of autonomy, you'll manage complex psychosocial presentations while supporting patients, families and the multidisciplinary team to achieve the best possible outcomes.
In this role you will be:
Delivering expert psychosocial assessment, counselling, advocacy, crisis intervention and discharge planning for patients with complex health and social needs.
Providing clinical leadership, consultation and specialist advice across Acute Stroke, Rehabilitation and Geriatric Medicine services.
Partnering with medical, nursing and allied health teams to coordinate safe, high-quality patient care in complex and high-risk situations.
Mentoring and supervising social workers and students while contributing to education, clinical supervision and evidence-based practice initiatives.
Supporting service development through quality improvement, policy implementation and continuous evaluation of clinical services.
